For me to bill your auto insurance carrier for injury rehab massage services related to your accident I must have a Doctors referral for massage therapy.   A doctors referral can be obtained from your Physical Therapy Rehab center, your Chiropractor, or your Medical Doctor  I have several highly skilled Physical therapists, Chiropractors and Medical Doctors I work with if you do not already have a doctors prescription for massage therapy

CANCELLATION POLICY
You must contact me by 8pm the day before your appointment or it will be considered insufficient cancellation notice and you will be charged.
Canceling without enough notice
1st time -- Warning
2nd time -- You pay 1/2 the price of the canceled appointment
3rd and future times -- You pay the full price of the canceled appt.

Canceling without enough notice does not allow me to fill the time you booked.
Please respect my time.  There are no exceptions.  If you cancel with insufficient notice I will charge you for the time you reserved.  By booking an appointment you agree to my cancellation policy.
